After a Discharge From Community Medical Center or Encompass Health
Community Medical Center at 99 Route 37 West opened in 1961 as Community Memorial Hospital and is now a 617-bed RWJBarnabas Health teaching hospital. It is the largest and most active health care facility in Ocean County, and its emergency department is one of the largest providers of emergency care in New Jersey. If a Toms River parent goes to the hospital, this is almost always where they go.
A short walk away at 14 Hospital Drive is Encompass Health Rehabilitation Hospital of Toms River, an inpatient rehabilitation hospital where patients receive three hours of therapy a day, five days a week, alongside 24-hour nursing. Others land in the township's sub-acute and skilled nursing facilities: Rose Garden Nursing and Rehabilitation Center and Complete Care at Bey Lea on Old Freehold Road, Complete Care at Green Acres on Lakewood Road, Hampton Ridge Healthcare and Rehabilitation on Stevens Road.
Wherever they were, the cliff is the same. One Tuesday there are three supervised hours of therapy, meals on a tray and a call bell. The next Tuesday there is a house, a walker that is new to everybody, a folder of instructions, and an adult child who has already used most of their time off.

Holiday City, Silver Ridge, and the Toms River Mailing Address
Holiday City and Silver Ridge Park form one enormous age-restricted development off Route 37 just west of the Garden State Parkway, threaded by Mule Road and Jamaica Boulevard. Holiday City alone is five separate homeowner associations, Berkeley, Carefree, West, South and Holiday Heights, and Silver Ridge Park adds four more: East, West, the Westerly Extension and North.
Here is the thing nobody tells families until it is inconvenient. Much of that development lies inside Berkeley Township even though the mail reads Toms River, and its zip code is 08757. Services that go strictly by municipality will tell you that you are not in Toms River. We cover 08753, 08754, 08755 and 08756 as well as 08757, so where the line falls on a tax map does not decide whether a caregiver shows up.
Holiday City at Silverton, by contrast, genuinely is Toms River, in the Silverton section: close to 1,600 single-family and duplex homes from the early 1970s. Greenbriar Woodlands, gated, holds roughly 1,250 single-family homes built between 1986 and 1998 around an 18-hole golf course. Between them, these communities are not a niche in the Toms River housing market; they are one of its largest components, which is exactly why home care demand here does not look like anywhere else in the county.

One Floor, One Bathroom, and a 1970s Doorway
Toms River's active-adult housing has one enormous advantage for home care and one recurring problem, and both come from the same decade.
The advantage: almost none of it has stairs. Holiday City homes are detached ranch-style houses on lots of at least fifty by one hundred feet. Silver Ridge homes are single-story on a crawl space with an attached garage on sixty-by-one-hundred-foot lots. Holiday City at Berkeley alone offers twenty single-story floor plans running from roughly 757 to 1,852 square feet; Silver Ridge's five plans run from 1,082 to 1,514 square feet, with one or two bedrooms and one or two bathrooms.
The problem: those bathrooms were drawn in the 1970s. A step-over tub, a single small bathroom, and a doorway sized before anybody was thinking about walkers. Nine times in ten the actual safety hazard in one of these houses is not the house, it is a five-foot square of tile where there is no room for a caregiver to stand beside someone rather than behind them. Grab bars, a transfer bench and a shower chair change more here than any renovation.
Greenbriar Woodlands homes from the late 1980s and 1990s invert the problem. They are larger, often on wooded lots, with sunrooms and fireplaces, which means more distance to cross at 2 a.m. and more house to keep track of for someone with memory loss. Ortley Beach, the Bridges, and the Summer Calendar
Toms River Township does not stop at the mainland. Ortley Beach, also called Dover Beaches South, is part of the township and sits on the Barnegat Peninsula between Lavallette and Seaside Heights, with Shelter Cove Beach on the bay side back on the mainland.
Getting there is the whole story. Route 37 crosses Barnegat Bay on a pair of bridges: the Thomas A. Mathis Bridge eastbound, a 1950 bascule span that still lifts for boat traffic, and the J. Stanley Tunney Bridge westbound, a high-level span opened in 1972. Route 37 is one of the very few links to the barrier island, it meets the Garden State Parkway at the Exit 82 cloverleaf, and it is routinely jammed with local and vacation traffic from June through Labor Day.
So an 8 a.m. shift at an Ortley Beach house in July is a different job from the same shift in February, and pretending otherwise is how a family ends up with a caregiver who is reliably twenty minutes late all summer. Off the island, the same logic applies to the county's transportation. Ocean Ride's Reserve-A-Ride program provides door-to-door non-emergency medical transportation for residents 60 and over and adults with disabilities, booked at 732-736-8989, but the driver may arrive anywhere in a one to one-and-a-half hour window and is not permitted to enter the home. Somebody has to have your mother dressed and waiting at ground level. That, in practice, is a large share of what a Toms River shift consists of.
