Mercer is a small county that behaves like several different places. It has just twelve municipalities, fewer than any other county in New Jersey except Hudson, and they range from Trenton, the state capital and the county seat, to the farmland and ridgeline of Hopewell Township, to the commuter subdivisions of West Windsor, East Windsor and Robbinsville along the Northeast Corridor. Coming Home From a Mercer County Hospital

Most families reach us in the days around a discharge, so it helps to know the local map. Mercer County's acute-care hospitals are Capital Health Regional Medical Center in Trenton, which holds the county's Level II trauma center, a state-designated comprehensive stroke center and Mercer's designated emergency mental health services center; Capital Health Medical Center – Hopewell, on One Capital Way in Pennington; and Robert Wood Johnson University Hospital Hamilton.

Princeton-area families usually end up just over the county line instead, at Penn Medicine Princeton Medical Center on Plainsboro Road in Plainsboro, with short-term rehabilitation next door at Merwick Care and Rehabilitation Center. And since Capital Health's East Trenton campus, the former St. Francis Medical Center, stopped taking emergency patients in June 2025, families in that part of the city are now traveling to Regional, Hopewell or RWJ Hamilton instead.

Wherever the discharge happens, the first two weeks at home are where readmissions start. That is what hospital-to-home care is for, alongside mobility and transfer support, personal care, Alzheimer's and dementia care and Parkinson's care.

If transport is the immediate problem rather than care, Mercer County TRADE runs free non-emergency rides for residents 60 and over and adults with disabilities, county-wide and up to five miles past the county line, on 609-530-1971. It will not cover the shower, the stairs or the missed meals. That is the part we staff.
