Two Counties That Need Different Things

Somerset and Hunterdon sit side by side and are run as one territory from our office at 220 Davidson Avenue in Somerset, but home care works differently in each.

Somerset County has 21 municipalities, a county seat in Somerville, and about 345,000 residents as of the 2020 census. Its land tilts from lowlands in the east along the Raritan River to hilly piedmont in the west, which in practice is the difference between a garden apartment off Easton Avenue and a house up a driveway on Schley Mountain. Roughly 19 percent of the county is 65 or older, by American Community Survey estimates.

Hunterdon is the opposite in almost every dimension. It is smaller, with about 129,000 residents at the 2020 census, but older: close to 23 percent of the county is 65 or over, one of the highest shares in New Jersey, and a majority of residents live in areas the Census Bureau classifies as rural. Its county seat is Flemington, and its towns are farm towns spread across the rolling ground above the Delaware River. Distance, not density, is the constraint on care there. That is why we tell Hunterdon families up front which shifts we can reliably staff instead of promising a schedule and rearranging it later.

The Hospitals Behind Most of Our Calls

Most people find us in the days after a hospital stay, so it helps to know where those stays happen.

Robert Wood Johnson University Hospital Somerset, at 110 Rehill Avenue in Somerville, is the anchor for the Somerset side: a 355-bed regional medical center with centers of excellence in cardiovascular care, cancer, stroke and joint replacement, the state's primary stroke center designation for the county, and Magnet recognition for nursing excellence four consecutive times. Hunterdon Medical Center, at 2100 Wescott Drive in Flemington, plays that role in the west, a 184-bed not-for-profit teaching hospital and designated stroke center at the head of a network of more than thirty practices across Hunterdon, Somerset, Mercer, Morris and Warren counties.

Around the edges, the map splits. Families in Bernards Township, Bernardsville and Bedminster are usually discharged from Atlantic Health's Morristown or Overlook Medical Centers, and veterans there often come home from the VA's Lyons campus inside Bernards Township itself. Families in Franklin Township go to RWJUH New Brunswick or Saint Peter's University Hospital. Families in Montgomery go south to Penn Medicine Princeton Medical Center. Wherever the discharge happens, the gap is the same one, and it is what our hospital-to-home care and stroke recovery care are built to close, usually with a same-day start.

On the Somerset side that runs from Basking Ridge, Bernardsville, Far Hills, Peapack and Gladstone in the Somerset Hills, through Bridgewater, Somerville, Raritan, Bound Brook, Manville and Martinsville along the Raritan, down through Hillsborough, Belle Mead, Skillman, Blawenburg and Rocky Hill, and out to Somerset, Franklin Park and Kingston in Franklin Township. On the Hunterdon side it covers Flemington, Clinton, Annandale, Whitehouse Station, Readington, Oldwick, Lebanon, High Bridge, Califon and Glen Gardner, and reaches the Delaware at Lambertville, Stockton, Frenchtown, Milford and Bloomsbury.

What Home Care Here Actually Includes

Always Responsive Home Care is a private-pay, non-medical agency founded by Teresa Sajkowski, a geriatric registered nurse with more than twenty years in hospital, hospice and home care. Our caregivers help with bathing, dressing, meals, transfers, fall prevention, medication reminders and companionship. We do not provide skilled nursing or therapy; we work alongside the clinicians who do.

Scheduling starts at a few hours a week with hourly care and goes up through overnight care to full live-in care. When memory is the reason you are calling, start with Alzheimer's and dementia care; when mobility is, start with mobility care; when a family caregiver is running out of road, start with respite care.

Both counties also run public aging services worth knowing about: the Somerset County Office on Aging and Disability Services, which operates the NJ EASE intake line, senior wellness centers and curb-to-curb paratransit for residents 60 and over, and the Hunterdon County Division of Senior, Disabilities and Veterans' Services. We use them the way families do, as a layer underneath private care rather than a substitute for it.
