What Is Mobility Care?

Mobility care is hands-on help with moving safely at home: getting out of bed or a chair, walking with a cane or walker, using a wheelchair, getting on and off the toilet, and repositioning in bed. Our caregivers are trained in safe transfers and in equipment such as gait belts, transfer boards, and Hoyer lifts.

It is non-medical care, and it works alongside therapy rather than replacing it. Brooks Rehabilitation lists geriatric rehabilitation and balance and vestibular therapy at outpatient clinics across our area: at Baptist HealthPlace at Nocatee on Colonnade Drive, on A1A North in Ponte Vedra Beach, on Southpark Boulevard in St. Augustine, on Pine Cone Drive in Palm Coast, on West Granada Boulevard in Ormond Beach, and at its San Pablo clinic on Beach Boulevard. It also runs an Outpatient Clinic and Balance Center on Centurion Parkway, in ZIP code 32256. A caregiver can get your loved one to those sessions and help keep up the walking and exercises the therapist recommends in between.

Trouble getting around is common across the area we serve. By the Census Bureau's 2020–2024 American Community Survey estimates, about 65,000 people 75 and older live in our ZIP codes, and a little more than one in four of them have difficulty walking or climbing stairs. In St. Augustine's 32084, Palm Coast's 32164 and Ormond Beach's 32174, it is about one in three.

Why Mobility Care Matters

Falls are the leading cause of injury for adults 65 and older, according to the Centers for Disease Control and Prevention. About one in four older adults reports a fall each year, and falling once doubles the chance of falling again.

At home, the risky moments are ordinary ones: getting out of bed at night, stepping out of the shower, standing up from a low chair. A caregiver there for those moments, with a steady hand and the right technique, lets your loved one keep moving instead of becoming afraid to.

How We Set Up Mobility Care

Every mobility plan starts with a look at the home and the way your loved one moves through it.

1

Free In-Home Assessment

We walk through the home with you, noting stairs, rugs, the bathroom, and the transfers your loved one finds hardest.

2

Written Care Plan

Before care starts, you get a written plan covering transfers, walking, equipment, and any exercises from your loved one's therapist.

3

Caregiver Match

We match a screened, insured caregiver trained in safe transfers and in the equipment your loved one uses.

4

Ongoing Support & Adjustments

As strength improves or declines, we adjust the plan, and our registered nurse is available for clinical questions.

Frequently Asked Questions

How is mobility care different from physical therapy?

A physical therapist evaluates your loved one, designs a program, and treats them in sessions, either at an outpatient clinic such as one of Brooks Rehabilitation's, or at home through a home health agency. Mobility care is the non-medical help in between: safe transfers, walking with your loved one, and encouraging the exercises the therapist recommends. The two work best together.

Can you help after hip or knee surgery?

Yes. After a joint replacement or a fall, the first weeks home are when steady help with getting up, walking, bathing, and using the bathroom matters most. We plan the visits around the discharge instructions and your loved one's therapy schedule. Call us at (904) 204-2046 before the discharge date if you can.

Where can we get grab bars or a ramp installed in St. Johns County?

The St. Johns County Council on Aging's Care Connection program sends volunteers to do minor home repairs and install grab bars and ramps; call (904) 209-3649 to ask about it. During the in-home assessment we point out the places where a grab bar, a cleared path, or better lighting would help most.
