Care Designed to Match Our Clients' Needs
Live-In Care
A live-in caregiver stays in the home and shares its routine, giving a parent steady help with bathing, dressing, meals and getting around from the first cup of coffee to bedtime. It suits someone who should not be alone for long stretches but wants to stay in the house, and it keeps the same familiar faces at the door week after week. In hurricane season it also means someone is already in the house when a storm watch is posted.
Hourly Care
Hourly care sends a caregiver for the hours that matter most, from a few hours a week to longer daily shifts: a shower and breakfast, a ride to an appointment at Baptist HealthPlace at Nocatee, UF Health Flagler Hospital in St. Augustine or Mayo Clinic, or lunch and company in the afternoon. The schedule can grow as needs change, all the way to around-the-clock coverage.
Overnight Care
An overnight caregiver stays awake in the house while your parent sleeps, ready for the bathroom trips, the restless hours and the early-morning routine. The person who usually keeps watch, often a spouse, gets to sleep through the night again.
Care Services Our Caregivers Are Trained to Provide
Personal Care
Personal care is hands-on help with bathing, dressing, grooming, toileting and moving safely around the house, given in a way that protects privacy and dignity. Caregivers follow the written plan from the free in-home assessment, so a parent in Nocatee, downtown St. Augustine or Flagler Beach gets the same careful routine at every visit.
Mobility Care
Mobility care covers safe transfers, walking with a cane or walker, and getting in and out of bed, the shower and the car, along with clearing the rugs and cords that cause falls. It pairs well with outpatient therapy: Brooks Rehabilitation has clinics in Nocatee, Ponte Vedra Beach, St. Augustine, Fleming Island, Palm Coast and Ormond Beach that offer geriatric rehab and balance therapy, and our caregivers can drive clients to those sessions and keep the routine going at home.
Companion Care
Companion care is for the parent who manages most things alone but no longer drives, or has lost the company that filled the day. A companion runs errands, shares a meal, helps with light housekeeping and gets your parent to the senior center: bridge or Tai Chi at THE PLAYERS Community Senior Center in Ponte Vedra Beach, lunch at the Council on Aging’s center in Hastings, or the Flagler County Senior Center in Bunnell. No Sunshine Bus fixed route reaches Nocatee or Ponte Vedra Beach, and Flagler County’s public transportation is a shared door-to-door service that riders must apply for and book ahead, so that ride is often the whole point.
Specialized Complex Care
Some clients need several kinds of help at once: a stroke on top of diabetes, heart failure after a long admission, or a long medication list from more than one specialist. Specialized complex care builds one non-medical plan around all of it, with consistent caregivers and a registered nurse on staff for the team to turn to when a client’s condition changes.
Alzheimer’s and Dementia Care
Memory loss changes what a good day looks like, so dementia care starts with routine: the same caregivers where possible, meals and bathing at familiar times, watchful company through the late afternoon and evening, and patience with repeated questions. We also point families to local programs, such as the Council on Aging’s Sunshine Center adult day care in St. Augustine, its Ponte Vedra Memory Care Center on A1A North, and the free monthly Memory Cafés around St. Johns County. ElderSource’s HelpLine, 1-888-242-4464, covers every ZIP code we serve.
Hospital to Home Care
Coming home from the hospital usually means new prescriptions, a walker and a list of follow-up appointments. Depending on the town, the hospital may be UF Health Flagler Hospital in St. Augustine, AdventHealth Palm Coast or AdventHealth Palm Coast Parkway, Baptist Medical Center Beaches in Jacksonville Beach, Baptist Health’s hospital on Fleming Island, Ascension St. Vincent’s St. Johns County off County Road 210, Baptist Medical Center South or Mayo Clinic in Jacksonville, or AdventHealth Daytona Beach or Halifax Health Medical Center, both in Daytona Beach; some parents come home after a stay at Brooks Rehabilitation. A caregiver can be at the house when your parent arrives, set up the bedroom and bathroom for safer moving around, keep meals and medication reminders on track, and drive to the follow-up visits.
Palliative Care
Palliative care at home is about comfort during a serious illness: help with bathing, eating and rest, gentle company through long days, and relief for the family members carrying the most. Our caregivers give non-medical support alongside any hospice or palliative medical team already involved, so the care at home follows their plan.
Parkinson’s Home Care
Parkinson’s care at home is built around timing and movement: medication reminders on schedule, steady help through freezing and transfers, and meals and outings planned around the hours when a client moves best. Mayo Clinic in Jacksonville is an American Parkinson Disease Association Center for Advanced Research and Brooks Rehabilitation treats Parkinson’s at its rehab hospital; our caregivers support the routine those teams set.
