Hospital-to-Home Care
A Safer, Steadier Start at Home After a Hospital Stay
What Is Hospital-to-Home Care?
Hospital-to-home care is non-medical help for the first days and weeks after a hospital or rehab stay: the ride home, a house set up for someone who is unsteady, meals, medication reminders, help with bathing and dressing, and someone there through the first night. We provide it in 28 ZIP codes across St. Johns, Flagler and Duval counties, from the Jacksonville Beaches and Fleming Island south through Ponte Vedra, Nocatee and St. Augustine to Palm Coast, Ormond Beach and Pierson.
The hospitals nearest Ponte Vedra are spread across two counties. Ascension St. Vincent's St. Johns County, at I-95 and County Road 210, is about 6 miles from our Nocatee office, and Baptist Medical Center South is about 9 miles away on Old St. Augustine Road.
Several more hospitals sit inside the ZIP codes we serve. UF Health Flagler Hospital, with 335 beds, is at 400 Health Park Boulevard in St. Augustine. Palm Coast has two AdventHealth hospitals, AdventHealth Palm Coast on Memorial Medical Parkway and AdventHealth Palm Coast Parkway. Baptist Medical Center Beaches is on 13th Avenue South in Jacksonville Beach, and Mayo Clinic Hospital is on San Pablo Road in Jacksonville. We plan the trip home from any of them, including the long ones: Mayo to Palm Coast City Hall is about 66 miles, roughly an hour and 20 minutes in free-flowing traffic.
Some people come home by way of inpatient rehab first, at Brooks Rehabilitation's University and Bartram campus hospitals in Jacksonville, or, for residents of Vicar's Landing, a short-term stay in the community's own skilled nursing wing. Call us before the discharge date and we will plan care to begin the day your loved one comes home.
Why the First Weeks Home Matter
The first weeks after a hospital stay are when people are weakest and the instructions are newest: new medications, a walker, a list of follow-up visits. A caregiver who is there through those weeks handles the basics, notices when something seems off and tells the family, and keeps the follow-up appointments on track.
Getting to those appointments takes planning. No Sunshine Bus fixed route reaches Ponte Vedra or Nocatee, so most follow-up visits there mean a car. Near our office, our caregivers can drive to primary care at Baptist HealthPlace at Nocatee on Colonnade Drive or UF Health Nocatee on Town Plaza Avenue, and to therapy at Brooks Rehabilitation's outpatient clinics in Nocatee and on A1A in Ponte Vedra Beach.
Our Hospital-to-Home Process
We start before discharge so the first day home goes smoothly.
Call Before Discharge
Tell us the hospital, the expected discharge date and what the care team has said about recovery. We schedule a free in-home assessment around that date.
Written Care Plan
Before care begins, you get a written plan covering the ride home, the first night, a medication reminder schedule taken from the discharge papers, bathing, meals and follow-up visits.
Caregiver Match
We match a caregiver to the recovery and the schedule: a few hours a day, overnight help, or live-in care for the first weeks.
Adjust as Recovery Goes
As strength returns, hours can come down. If something changes, our registered nurse on staff reviews the plan and the family hears from us.
Benefits of Hospital-to-Home Care
A steadier recovery and more confidence at home.
Discharge Planning
We work from the discharge instructions, and with the hospital's discharge planner when the family asks, so the move home is organized.
Medication Reminders
Reminders on the schedule in the discharge papers, so new prescriptions are not missed. Caregivers remind; they do not administer.
Mobility Support
Steady help with transfers, walking, and the walker or cane the therapist recommended.
Home Safety Setup
Clear paths, essentials within reach and a safer bathroom before the first night home.
Rides to Follow-Ups
Transportation to primary care, specialist and therapy visits, close to home or in Jacksonville.
Family Updates
Regular updates for family nearby or out of state, and a phone line answered at any hour.

Frequently Asked Questions
Does Medicare pay for help at home after a hospital stay?
Medicare may cover a limited period of skilled home health care, such as nursing or therapy visits, when a doctor orders it. It generally does not pay for non-medical help like ours: the hours of personal care, meals and supervision in between. We are private pay and accept long-term care insurance, with no deposits or contracts.
Can meals be delivered after a hospital stay?
In St. Johns County, the Council on Aging's Meals on Wheels program, (904) 209-3693, offers private-pay and temporary post-hospital options as well as subsidized meals for people 60 and older who qualify. Wherever your parent lives, our caregivers can also shop and cook to the diet the hospital recommends.
How far are the hospitals from Ponte Vedra?
By free-flow driving estimates from our office at Nocatee Town Center, Ascension St. Vincent's St. Johns County is about 6 miles away, Baptist Medical Center South about 9, Baptist Medical Center Beaches about 14, Mayo Clinic about 16 and UF Health Flagler Hospital about 22. Real trips often take longer.
Do you work with home health agencies?
Yes. If the hospital orders home health nursing or therapy, those visits come from a home health agency. Our caregivers fill the hours in between, help with the exercises the therapist leaves, and tell the family what they notice.
