What Is Specialized & Complex Care?

Specialized care is non-medical help at home for people living with serious or overlapping conditions, such as Parkinson's, multiple sclerosis, recovery after a stroke, ALS, heart failure, or several diagnoses at once. We provide it in 28 ZIP codes across St. Johns, Flagler and Duval counties, from Atlantic Beach south through Ponte Vedra, Nocatee and St. Augustine to Palm Coast, Ormond Beach and Pierson, and west across the St. Johns River to Fleming Island.

Complex care here often spans several health systems: a specialist at Mayo Clinic in Jacksonville, a primary care doctor near home, therapy at Brooks Rehabilitation, and hospital stays at Baptist, Ascension St. Vincent's, UF Health Flagler Hospital in St. Augustine or one of AdventHealth's two Palm Coast hospitals. Our caregivers keep the calendar, drive to appointments, write down what changed, and make sure the family knows.

Our caregivers do not provide nursing or medical treatment. Our registered nurse on staff gives each plan a clinical eye and is there for caregivers when a client's condition does not look right; nursing and therapy come from the medical team.

Why Complex Care Can Stay at Home

Many people would rather live with a serious condition in their own home, and with the right daily help many can, even as needs grow. Most of the work is steady and practical: safe transfers, meals that fit the diet, medication reminders on schedule, and someone who notices changes early.

Coordination is the hard part. Near our office, Baptist AgeWell Center for Health in Nocatee, at 98 Nocatee Village Drive, offers geriatric assessment, care coordination and medication management, and UF Health Nocatee on Town Plaza Avenue offers primary care, neurology, heart and vascular care, imaging and labs. Wherever a client's doctors are, with the family's permission, we share what the caregiver sees at home with their teams.

Our Specialized Care Process

A plan that fits the diagnosis, the medical team and the family, and changes when they do.

1

Free In-Home Assessment

We visit the home, list each condition, doctor and standing appointment, go over the medication schedule with the family, and look at the house for transfer and fall risks.

2

Written Care Plan

Before the first shift, a written plan sets out daily tasks, the reminder schedule, who to call at each practice, and what the caregiver reports to the family.

3

Caregiver Match

A caregiver with experience in the condition, matched to the person and the schedule, from a few hours a day to live-in care.

4

Ongoing Review

Our registered nurse on staff reviews the plan after any hospital stay, fall or new diagnosis, and the family hears from us regularly.

Frequently Asked Questions

Do your caregivers provide skilled nursing?

No. Our caregivers provide non-medical care: personal care, help moving safely, meals, medication reminders, supervision and rides to appointments. Nursing tasks such as injections, wound care and IV therapy come from a home health agency or the medical team, and our caregivers work around those visits.

Who coordinates with my parent's doctors?

You stay in charge of medical decisions. With your permission, we pass along what the caregiver sees at home, keep appointments on the calendar and drive to them. For a fuller review, Baptist AgeWell Center for Health in Nocatee (904-202-4243) offers geriatric assessment and care coordination.

What if my parent relies on oxygen or powered medical equipment during a hurricane?

Plan before the season starts on June 1, and plan with your parent's county, because each county runs its own special needs shelter. St. Johns County's Special Medical Needs Shelter is for people who rely on electricity for medical equipment or need oxygen therapy, and pre-registration is required, online, by mail or at (904) 824-5550. The shelter cannot support hospital beds, dialysis, ventilators, tracheostomy care or Hoyer lifts, a caregiver must come along, and shelter staff do not give one-on-one care. Registering with Florida's statewide Special Needs Registry does not guarantee a shelter space either, so a plan to stay with family or friends outside the evacuation zone matters too.

How does special needs shelter registration work in Flagler and Duval counties?

Each county's emergency management office handles its own registrations. Flagler County Emergency Management takes registrations online or on a paper form and answers questions at (386) 313-4200. In Duval, the City of Jacksonville's Emergency Preparedness Division uses the Florida Special Needs Registry, requires registering again every year and helps at (904) 255-3172. Both expect a caregiver or family member to come to the shelter, and Flagler's shelter staff cannot give one-on-one care.
