Home Care in Metuchen, NJ

Metuchen grew around its railroad, and the oldest parts of it still show it. Development along Main Street, Amboy Avenue and Woodbridge Avenue produced houses dating from the late 1840s through the 1890s, and the borough's own historic survey catalogs what came of it: Second Empire and Italianate houses from the 1860s and 1870s, Queen Anne and Shingle, Colonial Revival, American Foursquare, Craftsman and Tudor, concentrated in and around the Middlesex Avenue and Woodwild Park historic district.

These are beautiful houses and they are difficult houses. A caregiver's first week in one of them tends to revolve around the same handful of problems:

• A single full bathroom on the second floor, at the top of a staircase that is steeper and narrower than anything built after 1950.
• Doorways too narrow for a walker to pass squarely, so every trip becomes a turn.
• A basement laundry down a second, worse set of stairs.
• Rooms that were never meant to hold a hospital bed, which is a real conversation about the dining room long before it is a conversation about a nursing home.

We do the assessment inside the house for that reason. In a two-story Victorian, live-in care or overnight care is often the honest recommendation, because the stairs do not stop being stairs at 2 a.m. In a single-floor condominium closer to the station, a few hourly shifts a week can be genuinely enough.

Metuchen runs a real senior center, and families should use it. It sits at 15 Center Street, opens Monday through Friday from 9 a.m. to 3 p.m., and membership is free for borough residents aged 60 and over. The weekly schedule is genuinely full: chair exercise and chair yoga, Zumba Gold, line dancing, bingo, mahjong, cards, a walking club, monthly luncheons, health screening, benefits counseling, and a shuttle bus that runs by appointment on weekdays with the last pickup at 2:30 p.m.

Two things it cannot do. It closes at three, and the hours that frighten families most are the ones after that. And it is a place your parent has to be able to get to and move around in without help. Once that stops being true, the center goes from a resource to a reminder.

That is the seam we work in. A caregiver can be the reason your father still gets to Center Street on a Tuesday, and the reason there is someone in the house at eight o'clock that night. On the better days, the Middlesex Greenway is close by too: a flat, ten-foot-wide paved trail that starts at Greenway Park on Middlesex Avenue and runs three and a half miles through Edison to Woodbridge, with borough entrances at Rose Street, Amboy Avenue, Memorial Parkway, Main Street and Green Street. There are very few places in central New Jersey where somebody with a walker can go that far without meeting a curb.

County-level help is worth knowing about as well. The Middlesex County Office of Aging and Disabled Services in New Brunswick runs home-delivered meals, insurance counseling and a resource directory for caregivers, and the county's RIDE service takes reservations for residents 60 and over.

There is no standard package. What there is, is a small number of shapes that recur:

• A few shifts a week. Showers, laundry, groceries, a ride to the cardiologist. Personal care and companion care, usually a few months after a fall or a spouse's death.
• Nights only. An awake caregiver from evening to morning, most often when the second floor is the problem and the family has stopped sleeping.
• Live-in. One caregiver in the house day after day. This is what memory loss usually needs, and Alzheimer's and dementia care depends on the routine staying the same.
• Alongside hospice. Non-medical palliative care supports the household while the clinical team handles the medicine.

Medicare and traditional health insurance do not generally pay for non-medical home care, so this is private pay, by bank transfer or credit card, or through a long-term care insurance policy whose claim we will file for you.
