The Bayshore
Monmouth’s northern edge, along Raritan and Sandy Hook bays. Bayshore Medical Center at 727 North Beers Street in Holmdel is the hospital most families here are discharged from, and CareOne at King James in Atlantic Highlands takes a good share of the sub-acute rehab that follows.
Route 36 runs the length of this shoreline from Keyport out to Highlands. The North Jersey Coast Line stops at Aberdeen-Matawan, Hazlet and Middletown, and Seastreak sails to Manhattan from Belford, Highlands and Atlantic Highlands — the practical reason so many Bayshore adult children are ninety minutes from the front door when something goes wrong at home.
Housing matters here more than anywhere else in the county. Many homes were rebuilt elevated above base flood elevation after Superstorm Sandy, so the front door now sits at the top of an exterior staircase — the first obstacle in almost every Bayshore care plan.

The Two River Area
The towns wrapped around the Navesink and Shrewsbury rivers. Riverview Medical Center on Riverview Plaza in Red Bank is the anchor hospital, with Monmouth Medical Center at 300 Second Avenue in Long Branch a short run down Route 36 — the first hospital ever built in Monmouth or Ocean county.
Brookdale Community College is in Lincroft and Monmouth Park Racetrack in Oceanport. The North Jersey Coast Line is electrified only as far south as Long Branch, which makes Red Bank, Little Silver and Long Branch the practical rail stops for a family bringing a parent to an appointment by train.
Much of the housing here predates the war and runs to two and three stories, so a good share of the care planning is about the staircase between the living room and the bedroom.

The Shore Towns
The oceanfront strip and the towns immediately behind it, from Allenhurst south to Manasquan and Brielle. Jersey Shore University Medical Center at 1945 Route 33 in Neptune is the region’s Level I adult and pediatric trauma center; Jersey Shore Post Acute sits just off Route 18 nearby, and CareOne at Wall handles a large share of the sub-acute rehab further south.
Ocean Grove, inside Neptune Township, is a National Register historic district holding the largest assemblage of authentic Victorian architecture in the United States — beautiful houses, built as 19th-century summer cottages, on lanes barely wide enough for two cars. They were never designed for anyone to grow old in, which is exactly why in-home support tends to matter more here than a move does.
Rail service south of Long Branch requires a transfer there, with stops at Allenhurst, Bradley Beach, Belmar, Spring Lake, Sea Girt and Manasquan. And these towns empty out after Labor Day — worth weighing when a widowed parent’s nearest neighbor turns out to be a summer renter.

Western Monmouth
Our home ground. The office is at 78 Court Street in Freehold, the county seat, and CentraState Medical Center on West Main Street — Route 537 — is the hospital nearly everyone out here is discharged from. The Manor Health and Rehabilitation Center sits on the same CentraState campus, Monmouth Crossing Assisted Living is at 560 Iron Bridge Road, and Applewood Estates is on Applewood Drive, all within a few minutes of one another in Freehold.
The Monmouth County Division on Aging, Disabilities and Veterans Services — the county’s Area Agency on Aging — is also in Freehold, at 3000 Kozloski Road, (732) 431-7450.
This is where the county’s 55-plus housing sits: Covered Bridge and Four Seasons in Manalapan, Greenbriar in Marlboro. It is also where the county stops being suburban. Past the Route 9 corridor, Millstone, Perrineville, Cream Ridge, Upper Freehold, Allentown and Roosevelt are farm country, and the length of the drive to a hospital is a genuine factor in how a care plan gets written.
