What 32003 Covers, and Why It Runs Older

ZIP 32003 is Fleming Island: the unincorporated stretch of land that US 17 crosses between Orange Park and Green Cove Springs, with the St. Johns River on its east side, Doctors Lake to the north and Black Creek to the south. The Census Bureau’s ZIP code area for 32003 and its Fleming Island census-designated place cover the same 15.8 square miles of land, so the figures below describe the island itself, from Eagle Harbor and Pace Island to Fleming Island Plantation. The name goes back to George Fleming, an Irish immigrant who received a land grant here from the Spanish governor of East Florida for his military service and in 1790 established a plantation he called Hibernia. For our team, our services and how we work, start with home care across Ponte Vedra.

The Census Bureau’s 2020–2024 American Community Survey estimates describe an island that runs older. The median age in 32003 is 47.7. About 6,550 of its roughly 29,800 residents, 22.0%, are 65 or older, and about 550 are 85 or older. Four in ten households (39.6%) include someone 65 or older, and about 1,100 residents that age live alone. Among residents 75 and older, roughly three in ten report serious difficulty walking or climbing stairs, an approximate figure from a small sample. That is often the point where living alone stops working without help. Hourly care covers the meals, laundry and errands of an ordinary week a few hours at a time, personal care handles bathing and dressing when a parent would rather a professional helped than their own child, and mobility care means a steady arm on the stairs and in and out of the car. Our free in-home assessment walks through the actual house before care starts and ends in a written plan.

Coming Home From the Island Hospital, Orange Park or Middleburg

Fleming Island has its own hospital. Baptist Health’s hospital at US 17 and Village Square Parkway opened in December 2022 as the largest addition to a Baptist medical campus that dates to 2013. It has 102 beds, adult and pediatric emergency rooms open around the clock, and Primary Stroke Center certification, and in free-flowing traffic it is about 5 minutes’ drive from the Fleming Island library on Town Center Boulevard and about 7 from Eagle Harbor. HCA Florida Orange Park Hospital, on Kingsley Avenue, is about 15 minutes north by US 17: a 433-bed Level II Trauma Center and Comprehensive Stroke Center with a 42-bed inpatient rehabilitation center. Ascension St. Vincent’s hospital in Middleburg, with 134 beds and round-the-clock emergency care, is about 19 minutes west by County Road 220. For therapy after discharge, the Brooks Rehabilitation Outpatient Clinic at 4575 US 17 South, on the island, offers geriatric, neurologic, and balance and vestibular rehabilitation along with speech and occupational therapy.

Whichever hospital your parent leaves, the first days at home are when a family most needs a second pair of hands. Our hospital-to-home care is planned around the discharge date: prescriptions picked up on the way home, meals that follow the discharge instructions, help in and out of the shower, and a ride to the first follow-up. After a stroke or a hip replacement, mobility care carries the walking practice a therapist at Brooks asks for into the rest of the week, and for a parent seeing a neurologist about Parkinson’s, Parkinson’s care builds the day around medication reminders and the hours when moving is hardest. If the doctor orders nursing or therapy visits at home, our caregivers cover the hours in between. We are a non-medical, private-pay agency: we accept long-term care insurance, ask for no deposit or long-term contract, and don’t bill Medicare or Medicaid. There is a registered nurse on our staff.

Live-In Care

Live-In Care

Live-in care provides seniors with the consistency of having the same dedicated caregiver daily, allowing them to build trust and familiarity in their care routine. This arrangement enables seniors who need assistance with most activities of daily living to remain comfortably at home, maintaining their independence while receiving comprehensive support from their live-in caregiver.

Hourly Care

Hourly care provides flexible, scheduled support for seniors in their homes. Caregivers visit for set periods, offering assistance with daily tasks, personal care, and companionship tailored to the client's specific needs and preferences. This service can range from short shifts a few days a week to full 24/7 around-the-clock care, accommodating a wide spectrum of care requirements and ensuring seniors receive the right level of support.

Overnight Care

Overnight Care

Overnight care offers peace of mind with an awake caregiver present during nighttime hours. This service ensures assistance is available for nighttime needs, emergencies, or early morning routines, allowing clients needs to be met overnight, enabling family members or daytime caregivers to have uninterrupted sleep.

Always On Virtual Care

Always On Virtual Care

Always On Virtual Care pairs our caregivers with Sensi, an AI-powered audio sensor that detects falls, distress, early UTI signs, and other care events around the clock. Sensi alerts our team the moment something changes — so families gain a second set of eyes during the hours a caregiver isn't physically in the room, and seniors stay safer at home for longer.

Senior Centers, Buses and the Bridges Across the River

There is no senior center on the island itself. The two closest, both run by Aging True, are a short drive up and down US 17: the Orange Park Senior Center at 414 Stowe Avenue, about 11 minutes north, and the Clayton and Mildred Revels Senior Center at 604 Walnut Street in Green Cove Springs, about 16 minutes south, which also houses Aging True’s adult day care program. Both are open weekdays for people 60 and older, serve a hot meal every weekday, and charge nothing to attend. Getting there is the hard part for a parent who has stopped driving. Companion care takes on the drives, to the senior center, the pharmacy and the doctor, and on the days in between, Alzheimer’s and dementia care at home keeps the same routines going. ElderSource, the state-designated Area Agency on Aging for Northeast Florida, is the place to start for publicly funded help; its HelpLine is 1-888-242-4464.

Our office is at Nocatee Town Center in St. Johns County, on the other side of the St. Johns River. In free-flowing traffic it is about 43 minutes from Fleming Island, north on US 17 to I-295 and east across the Buckman Bridge, or about 53 minutes south through Green Cove Springs and over the Shands Bridge on State Road 16. Fruit Cove, part of our St. Johns and Julington Creek area, is only about five and a half miles from the Fleming Island library as the crow flies, yet the drive takes about 32 minutes because the road goes around by the Buckman Bridge. That distance is why your parent’s assessment happens at their house, not ours, and why we want to hear from caregivers who live on Fleming Island and nearby: see caregiver jobs in our area.

Doctors Lake, Black Creek and the River in Storm Season

Fleming Island has water on three sides, and Hurricane Irma showed what that means. In September 2017, Black Creek crested at a record 28.5 feet, breaking a mark of just over 24 feet set in 1923. Across the area, floodwaters, worst along Black Creek and Doctors Lake, heavily damaged or destroyed at least 450 homes and sent 900 residents to five emergency shelters, and later that month, with an astronomical tide holding the St. Johns River high, local officials were watching northern Fleming Island for more flooding. Evacuations here go by zone, and the evacuation map splits 32003 among five of them, A through E, so two houses on the island can get different orders. Ahead of Hurricane Dorian in 2019, Zones A and B were ordered to leave; for Hurricane Ian in 2022 the orders covered Zones A, B and C and the two Black Creek prong zones, and every mobile or manufactured home counts as Zone A wherever it stands. Look up your parent’s address on the Evacuation Zone Helper rather than going by the neighborhood.

Anyone whose care goes beyond the basic first aid a general shelter provides should preregister through Florida’s Special Needs Registry, as the Florida Department of Health’s local office advises. Registering helps reserve space for the people with the greatest need, but the special needs shelter has no complex medical equipment or care, and a parent who needs a caregiver’s help at home should bring that caregiver along. All of that argues for settling the plan before a storm is in the forecast. With live-in care, someone is already in the house when a watch is posted, gathering medications, chargers and paperwork and making it possible to leave early rather than at the last minute. When the plan is to stay, overnight care puts someone awake in the house through the storm, and for families who live out of town, extra hourly care shifts before and after it mean someone checks that the refrigerator is cold, the medications are dry and your parent is eating. Our phone at (904) 204-2046 is answered 24 hours a day. To see every ZIP code we cover, visit our Ponte Vedra service areas page.
