Two Southeast Jacksonville ZIP Codes, Not the Whole City
We serve two ZIP codes in Jacksonville, not the whole city. ZIP 32224 runs from the San Pablo area north of Beach Boulevard south across J. Turner Butler Boulevard, taking in the Mayo Clinic campus, the University of North Florida and the neighborhoods along Kernan Boulevard, Hodges Boulevard and San Pablo Road; a small piece of its southeast corner lies in St. Johns County. ZIP 32256 is the stretch below J. Turner Butler: Baymeadows and Deerwood, Sunbeam, the Gate Parkway and A. C. Skinner Parkway corridors and, farther south, eTown and Bayard. St. Johns Town Center, just north of J. Turner Butler, is in neighboring 32246 and outside our area. Both ZIP codes border 32081, where our office is, at 205 Marketside Avenue in Nocatee Town Center. From there, Baymeadows Road at Southside Boulevard is about 24 minutes’ drive in free-flowing traffic, and Kernan Boulevard at J. Turner Butler about 25. For the team, the services and how we work, start with home care across Ponte Vedra.
The Census Bureau’s 2020–2024 American Community Survey estimates describe two young ZIP codes by Duval County standards: the median age is 33.6 in 32224 and 34.4 in 32256, against 36.9 countywide. Young is not the same as having no older residents. About 16,100 people in the two ZIP codes are 65 or older, roughly 7,100 in 32224 and 9,000 in 32256, more than 2,000 are 85 or older, and about 3,700 of those 65 and older live alone. Living alone works until a fall, a new diagnosis or a hospital stay, and then a family has to answer two questions quickly: who will be there at night, and who will help during the day. Overnight care puts someone awake in the house after dark, and hourly care covers bathing, meals, laundry and errands a few hours at a time. We are open Monday through Friday from 8:30 a.m. to 4:30 p.m., the phone at (904) 204-2046 is answered around the clock, and there is a registered nurse on our staff. Care starts with a free in-home assessment and a written plan. We are private pay, accept long-term care insurance and don’t bill Medicare or Medicaid.

In 32224, Mayo Clinic Is the Neighborhood Hospital
Mayo Clinic opened its Jacksonville clinic in 1986, and Mayo Clinic Hospital, which has an emergency department, opened on the San Pablo Road campus in April 2008. Patients come from all 50 states and more than 80 countries, but from Hodges Boulevard at J. Turner Butler the hospital is about six minutes’ drive in free-flowing traffic, and about eight from Kernan Boulevard. Florida law makes Mayo in Jacksonville one of the state’s memory disorder clinics, for research and training in diagnosing and treating Alzheimer’s disease and related disorders, and it is one of the American Parkinson Disease Association’s Centers for Advanced Research. The campus also runs Advanced Care at Home, Mayo’s hospital-at-home program for eligible patients: a Mayo doctor holds a video visit every day, a nurse checks in by video about four times a day, and the patient is asked to stay home until care is complete. If your parent comes home from Mayo Clinic Hospital or is treated at home through that program, hospital-to-home care covers the hours between visits: help to the bathroom and shower, meals that fit the discharge instructions, clean sheets, and a call to you when something seems off. After a memory evaluation, Alzheimer’s and dementia care keeps the days at home calm and predictable, with the same faces and the same routine, and for a parent treated there for Parkinson’s, Parkinson’s care plans the day around medication reminders, slow mornings and a safe path to the bathroom at night.
The University of North Florida is in 32224 as well, and its Osher Lifelong Learning Institute offers non-credit classes designed for people 50 and older. A standing class on campus is the kind of weekly outing companion care keeps going once a parent stops driving. Brooks Rehabilitation’s San Pablo clinic, on Beach Boulevard just over the ZIP line in 32250, lists balance and vestibular, neurologic and geriatric rehabilitation among its services and is about eight minutes from Hodges at J. Turner Butler; a caregiver can drive, wait, and help with the walks the therapist assigns for home. Baptist Medical Center Beaches, with a 24-hour emergency room, is about 11 minutes east of the same corner; for the beach side, see home care in Jacksonville Beach. Cypress Village, a Life Plan Community on 120 wooded acres just off J. Turner Butler Boulevard, offers independent living, assisted living, memory care, and long-term care and rehab. For a family weighing a move like that, care at home keeps things steady while you visit, compare and decide.
Services offered in Jacksonville, FL
Live-In Care
Live-in care provides seniors with the consistency of having the same dedicated caregiver daily, allowing them to build trust and familiarity in their care routine. This arrangement enables seniors who need assistance with most activities of daily living to remain comfortably at home, maintaining their independence while receiving comprehensive support from their live-in caregiver.
Hourly Care
Hourly care provides flexible, scheduled support for seniors in their homes. Caregivers visit for set periods, offering assistance with daily tasks, personal care, and companionship tailored to the client's specific needs and preferences. This service can range from short shifts a few days a week to full 24/7 around-the-clock care, accommodating a wide spectrum of care requirements and ensuring seniors receive the right level of support.
Overnight Care
Overnight care offers peace of mind with an awake caregiver present during nighttime hours. This service ensures assistance is available for nighttime needs, emergencies, or early morning routines, allowing clients needs to be met overnight, enabling family members or daytime caregivers to have uninterrupted sleep.
Always On Virtual Care
Always On Virtual Care pairs our caregivers with Sensi, an AI-powered audio sensor that detects falls, distress, early UTI signs, and other care events around the clock. Sensi alerts our team the moment something changes — so families gain a second set of eyes during the hours a caregiver isn't physically in the room, and seniors stay safer at home for longer.
Hospitals, Rehab and Rides From Baymeadows and Deerwood
Several hospitals are a short drive from 32256. From Baymeadows Road at Southside Boulevard, Ascension St. Vincent’s Southside, a Primary Stroke Center with a 24/7 emergency room at 4201 Belfort Road, is about seven minutes away in free-flowing traffic, and about six from Deerwood Park Boulevard. Baptist Medical Center South, an Advanced Primary Stroke Center at 14550 Old St. Augustine Road, is about ten minutes, and Mayo Clinic about 15. When a hospital stay ends in inpatient rehab, Brooks Rehabilitation has two hospitals nearby: the 60-bed Bartram Campus off I-95, about 14 minutes south, on a 115-acre campus it shares with Brooks’s Bartram Crossing skilled nursing facility, and the 170-bed University Campus in San Souci, about 11 minutes north. The Bartram Campus is built for people who need intensive therapy after a stroke, a brain or spinal cord injury or another serious injury or illness, and it trains family members for what comes after discharge. What follows is ordinary life with less strength than before. Mobility care means a steady arm from bed to bathroom and out to the car, and personal care covers bathing and dressing for as long as they are hard to manage alone.
Renting is common here. About six in ten households in 32256 rent, and in both ZIP codes roughly a third of householders 65 and older do, against fewer than a quarter countywide. A renter can’t always add a grab bar or swap a tub for a walk-in shower, so more of the safety has to come from the help. Getting around without a car takes planning too. JTA’s fixed-route buses are free at 65 with a Florida driver’s license, a Florida ID or a Senior STAR Card. For a parent who can’t manage a bus stop, Connexion, JTA’s paratransit service, runs shared rides anywhere in Duval County for riders certified under the ADA or the Transportation Disadvantaged program; trips are booked one to seven days ahead, and the ADA fare is $3 each way. The city’s senior centers are open weekdays to anyone 60 or older, with a midday meal and door-to-door transportation on request. The Southside Community Center at 10080 Beach Boulevard is about ten minutes from Baymeadows at Southside or from Kernan at J. Turner Butler, and the Jim Fortuna Senior Center on McCormick Road about 15 from Kernan. On the days a parent would rather go with someone familiar, a caregiver drives, stays and brings them home.
Evacuation Zones Reach Inland Here
Duval County’s evacuation zones run from A, the most vulnerable and the first to be evacuated, through E, with Zone F marking areas likely to flood from rainfall or other hazards. They are not only a beach concern. On the city’s map, much of the San Pablo area north of Beach Boulevard is in Zones A and C, the Mayo Clinic campus is in Zone C, Hodges Boulevard at J. Turner Butler is in Zone E, and Zone A follows Pablo Creek through the eastern side of 32256, while Baymeadows Road at Southside Boulevard is in no zone at all. Zones are assigned address by address, so look up your parent’s on the city’s evacuation-zone search at maps.coj.net/evsearch, or call 630-CITY, rather than going by the neighborhood.
If your parent would need a public shelter and depends on oxygen or powered medical equipment, needs help with medications or has dementia, the city asks them to join its Special Medical Needs Registry well before a storm, online, by mail or at (904) 255-3172. Joining does not guarantee a place in a special needs shelter: Florida Department of Health staff review every application, and the registry must be renewed each year starting in January. Atlantic Coast High School, on R. G. Skinner Parkway inside 32256, is on the city’s list of shelters most accessible to people with disabilities, but no shelter is certain to open for a given storm, so check JaxReady or call 630-CITY before going. Live-in care means the person who helps your parent pack and leave is already living in the house when an order comes. For publicly funded help year-round, ElderSource, the state-designated Area Agency on Aging for Northeast Florida, is the place to start: its HelpLine, 1-888-242-4464, screens callers by phone for Medicaid’s long-term care program, and people who qualify go on a waitlist ordered by frailty and available funding. Private-pay home care can cover the time on that list. If your parent lives farther south, see home care in Nocatee and Ponte Vedra 32081 or home care in St. Johns and Fruit Cove, or find every ZIP code we serve on our Ponte Vedra service areas page.
