Nocatee, ZIP 32081, and Our Office at Town Center
ZIP 32081 is the Postal Service’s Ponte Vedra, and most of it is Nocatee. Of its roughly 35 square miles of land, about 22 fall in the Census area for Nocatee, about 3.4 lie inside the City of Jacksonville in Duval County, and the rest is unincorporated St. Johns County outside any named place. Nocatee itself spans northern St. Johns and southern Duval. Its developer, The PARC Group, describes about 25 square miles of Davis family land between the Intracoastal Waterway in Palm Valley and US 1, where ground was broken in 2005 along with Nocatee Parkway. The first residents moved in during 2006, in neighborhoods including Coastal Oaks, Del Webb Ponte Vedra and Austin Park; Splash Waterpark followed in 2010, and 2,400 acres along the Intracoastal were committed to the county’s Nocatee Preserve.
Our office is in the middle of it, at 205 Marketside Avenue, the Town Center street that also holds the Publix-anchored Nocatee Town Center shopping center. We are open Monday through Friday from 8:30 a.m. to 4:30 p.m., the phone at (904) 204-2046 is answered around the clock, and there is a registered nurse on our staff. Because Nocatee crosses a county line, some public services depend on which side the house is on. ElderSource, the state-designated Area Agency on Aging for both St. Johns and Duval counties, works on either side: its HelpLine, 1-888-242-4464, is the place to start for subsidized programs and for the screening for Medicaid’s long-term care program. We are private pay, accept long-term care insurance and don’t bill Medicare or Medicaid. For the team and everything else we offer, start with Always Responsive Home Care of Ponte Vedra.

A Young ZIP Code With Plenty of Parents in It
By the Census Bureau’s 2020–2024 American Community Survey estimates, 32081 is young for St. Johns County, with a median age of 41.7 across about 29,800 residents, against 44.2 countywide. That median hides a sizable older population. About 6,500 residents are 65 or older, 35.8% of households include someone that age, and about 825 people 65 and older live alone. A common situation here is a parent who moved to be near a son or daughter and settled a few streets away. It works well until a fall, a diagnosis or the end of driving, and then the work lands on an adult child with a job and a family of their own. Hourly care covers the weekday hours while you are at work, companion care takes on the drives, the grocery runs and the company, and personal care handles bathing and dressing when a parent would rather a professional helped than their own child.
Nocatee is a new community, but a new house is not automatically an easy one to grow old in. A primary bedroom up a flight of stairs, a step from the garage into the kitchen and a tub you have to climb into are ordinary features that turn into hazards after a hip replacement or a stroke. In the same survey, roughly a quarter of 32081 residents 75 and older report trouble walking or climbing stairs, an approximate figure from a small sample. Our free in-home assessment walks through the actual house before care starts and ends in a written plan, and mobility care covers what happens between the rooms: a steady arm on the stairs, help in and out of the car, and the daily walks a physical therapist asks for.
Services offered in Nocatee, FL
Live-In Care
Live-in care provides seniors with the consistency of having the same dedicated caregiver daily, allowing them to build trust and familiarity in their care routine. This arrangement enables seniors who need assistance with most activities of daily living to remain comfortably at home, maintaining their independence while receiving comprehensive support from their live-in caregiver.
Hourly Care
Hourly care provides flexible, scheduled support for seniors in their homes. Caregivers visit for set periods, offering assistance with daily tasks, personal care, and companionship tailored to the client's specific needs and preferences. This service can range from short shifts a few days a week to full 24/7 around-the-clock care, accommodating a wide spectrum of care requirements and ensuring seniors receive the right level of support.
Overnight Care
Overnight care offers peace of mind with an awake caregiver present during nighttime hours. This service ensures assistance is available for nighttime needs, emergencies, or early morning routines, allowing clients needs to be met overnight, enabling family members or daytime caregivers to have uninterrupted sleep.
Always On Virtual Care
Always On Virtual Care pairs our caregivers with Sensi, an AI-powered audio sensor that detects falls, distress, early UTI signs, and other care events around the clock. Sensi alerts our team the moment something changes — so families gain a second set of eyes during the hours a caregiver isn't physically in the room, and seniors stay safer at home for longer.
Senior Medicine Inside Nocatee
Much of the care an older adult needs is inside Nocatee itself. Baptist HealthPlace at Nocatee, at 400 Colonnade Drive off Nocatee Parkway, houses Baptist Primary Care, which lists geriatrics among its services, and Brooks Outpatient, which offers geriatric rehabilitation and balance and vestibular therapy. The Baptist AgeWell Center for Health at 98 Nocatee Village Drive is built around older patients, with geriatric assessment, care coordination, medication management, senior rehabilitation and memory disorder care. UF Health Nocatee, open since January 2021 at 351 Town Plaza Avenue, adds primary care, orthopaedics, imaging, labs, heart and vascular care and neurology.
Doctors nearby help only if someone can get your parent there, sit through the visit and remember what was said. Our caregivers drive to the appointment, wait, take notes for the family, pick up the prescription on the way home and tell you how it went. When a visit at AgeWell or UF Health ends with a memory diagnosis, Alzheimer’s and dementia care keeps the day predictable at home, and for a parent seeing a neurologist about Parkinson’s, Parkinson’s care builds the day around medication reminders and the hours when moving is hardest. For a parent who no longer drives, a caregiver can make the weekly trip to the Council on Aging’s THE PLAYERS Community Senior Center on Landrum Lane in Ponte Vedra Beach.
Hospitals Along I-95, and Getting Around Without a Car
From Town Center, Ascension St. Vincent’s St. Johns County, at 205 Trinity Way where I-95 meets County Road 210, is about 14 minutes’ drive in light traffic. It opened in July 2022 and is a Primary Stroke Center with a 24-hour emergency room. Baptist Medical Center South, a 381-bed hospital on Old St. Augustine Road at I-95, is about 16 minutes away, and Mayo Clinic in Florida, on San Pablo Road South, about 27; Florida law establishes one of the state’s memory disorder clinics at Mayo’s Jacksonville campus. Whichever one your parent comes home from, hospital-to-home care starts on discharge day, with someone in the house the first nights, meals ready, follow-up visits on the calendar and a ride to each. When living alone is no longer realistic, live-in care keeps a caregiver in the home around the clock.
Getting to all of it takes a car. None of the county’s Sunshine Bus fixed routes reach Nocatee, though the Council on Aging runs on-demand rides for people over 60 and for residents who qualify as transportation-disadvantaged. Senior living is growing here as well: Starling at Nocatee, across from Splash Waterpark, offers independent living, assisted living, memory care and respite stays. In-home care is how a parent stays in their own house instead. In storm season, the county’s evacuation route for Nocatee and Palm Valley runs southwest to County Road 210 and on to I-95. Being inland does not by itself put a house outside an evacuation zone, so check your parent’s address on St. Johns County’s evacuation information page or, on the Duval side, the City of Jacksonville’s JaxReady site.
We also serve the ZIP codes next door: Ponte Vedra Beach and Sawgrass to the east, St. Johns and Julington Creek to the west along County Road 210, Jacksonville’s 32224 and 32256 to the north, and St. Augustine, whose ZIP 32095 borders 32081 to the south. In all, the office covers 28 ZIP codes, from Atlantic Beach south to Ormond Beach and Pierson and west across the St. Johns River to Fleming Island; find yours on our Ponte Vedra service areas page.
