What 32259 Covers, From Switzerland to RiverTown
ZIP 32259 is the northwest corner of St. Johns County, running from the St. Johns River east past I-95, with the Duval County line to the north. Addresses here may read St. Johns or Jacksonville, but apart from less than a square mile that crosses into Duval, the whole ZIP is unincorporated St. Johns County, and that decides which Council on Aging, which evacuation zones and which county services apply to your parent. The Census Bureau’s Fruit Cove takes in about 16 of the ZIP’s 57 square miles of land, in its northern part: Switzerland, the river side of State Road 13, and Julington Creek Plantation, a planned community of about 5,800 homes on some 4,120 acres off Race Track Road. Toward I-95 are Durbin Crossing and the Pavilion at Durbin Park, and to the south is RiverTown, Mattamy Homes’ riverfront community off Longleaf Pine Parkway, which includes WaterSong, a gated neighborhood for buyers 55 and older. If your parent’s mail goes to a post office box in 32260, a ZIP code used only for boxes, they are in our area too. Our office is at Nocatee Town Center, about 19 minutes’ drive from Julington Creek Plantation and 27 from RiverTown in light traffic; for the team and everything we offer, start with home care across Ponte Vedra.
By the Census Bureau’s 2020–2024 American Community Survey estimates, 32259 is young for St. Johns County. Its median age is 39.1, against 44.2 countywide, and 11.2% of its roughly 75,000 residents are 65 or older, about half the county’s share of 21.1%. Only 22.9% of its households include someone 65 or older, against 36.4% across the county. That still adds up to about 8,400 older residents, close to 1,000 of them 85 or older. About 1,260 people 65 and older live alone, most of them women, and roughly 770 more live in a grown son’s or daughter’s household, an approximate figure from a small sample. Each arrangement has its own gap: a parent on their own needs someone to come by, and a parent living with a family that is out at work and school all day spends most of it alone. In either one, personal care puts bathing, dressing and grooming in the hands of a trained caregiver rather than a son or daughter, which many parents prefer.

Caring for a Parent Around the Commute North
Many of the adult children doing that caring also commute. In the same survey, about 41% of working residents of 32259 work outside St. Johns County, against 28.5% countywide, and in the Fruit Cove area the share reaches 47%. The average trip to work takes 29 minutes. JTA’s St. Johns Express Select commuter bus leaves the Pavilion at Durbin Park for downtown Jacksonville at 6:55 and 7:25 on weekday mornings and comes back between 5:20 and 7:45 in the evening, so for a commuter who rides it, the workday runs from before seven until after five. Hourly care fills those hours with a caregiver who makes lunch, gives medication reminders, does the laundry, goes along on a walk and tells you at the end of the day how it went. Working from home, as about three in ten working residents here do, helps less than it sounds when a parent needs help in the middle of a meeting. And when the hard part is the night, a parent up and wandering or unsteady on the way to the bathroom, overnight care puts someone awake in the house so the family can sleep before the next workday.
Getting a parent out of the house matters as much. The St. Johns County Council on Aging runs the Trout Creek Senior Center at 6795 Collier Road, just south of the ZIP, about 11 minutes’ drive from RiverTown and 22 from Fruit Cove. It is open to adults over 60 several days a week, with lunch, fitness classes, games, crafts and talks, and it takes transportation requests from people who can’t drive; the Council’s demand-response paratransit also carries riders over 60. Companion care turns outings like that into a weekly habit and keeps a parent company on the days in between, and when memory loss is the issue, Alzheimer’s and dementia care keeps the routine steady at home. Families weighing a move have options inside the ZIP. Westminster Woods on Julington Creek, at 25 State Road 13, is a Life Plan community with independent living, assisted living, memory support, rehabilitation and skilled nursing, and The Blake at St. Johns, on County Road 210, offers assisted living and memory care. Care at home keeps your parent settled while the family decides.
Services offered in St. Johns, FL
Live-In Care
Live-in care provides seniors with the consistency of having the same dedicated caregiver daily, allowing them to build trust and familiarity in their care routine. This arrangement enables seniors who need assistance with most activities of daily living to remain comfortably at home, maintaining their independence while receiving comprehensive support from their live-in caregiver.
Hourly Care
Hourly care provides flexible, scheduled support for seniors in their homes. Caregivers visit for set periods, offering assistance with daily tasks, personal care, and companionship tailored to the client's specific needs and preferences. This service can range from short shifts a few days a week to full 24/7 around-the-clock care, accommodating a wide spectrum of care requirements and ensuring seniors receive the right level of support.
Overnight Care
Overnight care offers peace of mind with an awake caregiver present during nighttime hours. This service ensures assistance is available for nighttime needs, emergencies, or early morning routines, allowing clients needs to be met overnight, enabling family members or daytime caregivers to have uninterrupted sleep.
Always On Virtual Care
Always On Virtual Care pairs our caregivers with Sensi, an AI-powered audio sensor that detects falls, distress, early UTI signs, and other care events around the clock. Sensi alerts our team the moment something changes — so families gain a second set of eyes during the hours a caregiver isn't physically in the room, and seniors stay safer at home for longer.
Hospitals on County Road 210 and Up I-95
The hospital inside 32259 is Ascension St. Vincent’s St. Johns County, at 205 Trinity Way off County Road 210, just east of I-95. It opened on July 6, 2022, as a 56-bed hospital with eight ICU beds, and it is a Primary Stroke Center with a 24-hour emergency room whose teams connect patients to cardiology, neurology, orthopedic and lung specialists for follow-up. In free-flowing traffic it is about 16 minutes’ drive from Julington Creek Plantation and 18 from RiverTown. Baptist Medical Center South, a 381-bed hospital in Duval County at I-95 and Old St. Augustine Road, is about 15 minutes from Julington Creek Plantation and 22 from Cunningham Creek Estates in Fruit Cove. The ZIP also has a 24-hour emergency room that is not a hospital: HCA Florida Julington Creek Emergency, at 42 Doctors Village Drive, an off-campus emergency department of HCA Florida Memorial Hospital in Jacksonville. Whichever hospital your parent leaves, hospital-to-home care starts on discharge day: the new prescriptions picked up on the drive home, meals that follow the discharge instructions, a steady hand on the way to the bathroom, someone in the house the first nights, and every follow-up visit on the calendar with a ride to each.
Much of the recovery can happen without leaving the ZIP. Brooks Rehabilitation’s Julington Creek outpatient clinic, at 450 State Road 13 North, treats orthopedic conditions, and Baptist Primary Care has three offices here: in Fruit Cove on State Road 13, at the Pavilion at Durbin Park, and at 1310 County Road 210 West. For a parent in therapy, mobility care means a steady arm from the car to the clinic door and help with the walking and exercises the therapist assigns for home. For a parent with Parkinson’s, Parkinson’s care schedules walks, meals and bathing for the hours when moving comes easiest, with medication reminders through the day. We are a non-medical, private-pay agency: we accept long-term care insurance and don’t bill Medicare or Medicaid, and if the doctor orders home health visits, our caregivers cover the hours in between. Care starts with a free in-home assessment and a written plan, and there is a registered nurse on our staff.
Storm Season on the River, and the Long Way Around It
The river that gives 32259 its views can also flood it. St. Johns County records that in September 2017 Hurricane Irma pushed storm surge 1 to 4 feet above ground level along the St. Johns River and caused significant power outages and tree damage across the county. The county’s evacuation map puts low ground near the river and Julington Creek in Zone A, and other land in Zone F, which it describes as an area along the St. Johns River that is vulnerable to flooding, while much of the ZIP inland is in no zone at all. The lines follow elevation, not neighborhood names, so look up your parent’s address with the county’s My Evacuation Zone tool. From State Road 13 north of County Road 210, the county’s routes run east on County Road 210 or Greenbriar Road to I-95, north on State Road 13 into Duval County, or east on Race Track Road. For someone who can’t drive, the county will take registered residents to a shelter, but only to and from shelters inside St. Johns County, at a pickup time the Emergency Operations Center assigns, and it warns that a missed pickup may not be repeated. Registration is open all year at (904) 824-5550, and the county says the best time is before hurricane season begins in June. With live-in care, someone is already in the house when that call comes, with medications, chargers and paperwork ready by the door, and still there after a storm that takes the power out, keeping meals, medications and the household on track.
The river is also why Fleming Island is farther away than it looks. It is about five and a half miles across the water from Fruit Cove, but no bridge connects them: from Fruit Cove the drive goes north over the Buckman Bridge on I-295, about 32 minutes in light traffic, and from RiverTown it goes south over the Shands Bridge on State Road 16, about 30. If you live on one side of the river and your parent on the other, we cover both; see home care in Fleming Island. For the area around our office, see home care in Nocatee and Ponte Vedra 32081, and if your parent lives up I-95 in Jacksonville, we serve two ZIP codes there: see home care in Jacksonville 32224 and 32256. Every ZIP code we cover is on our Ponte Vedra service areas page, and you can contact our team to set up a free in-home assessment.
