Palm Coast by ZIP Code, and Who Lives Here
Palm Coast’s homes fall in two ZIP codes. 32137 is the north side: Palm Harbor, the C and F Sections, where saltwater canals run out to the Intracoastal Waterway; Indian Trails, the B Section; Matanzas Woods, the L Section; and, across the Intracoastal, the Hammock, the strip of unincorporated Flagler County along Scenic A1A between the waterway and the ocean, home to Washington Oaks Gardens State Park. 32164 is the south side: Pine Grove, Lehigh Woods and Seminole Woods. The Postal Service also uses 32135, 32142 and 32143 in Palm Coast for post office boxes only; if your parent’s mail goes to one of them, we cover it too. ITT laid out the lettered sections in the late 1960s and early 1970s, with street names that match each section’s letter, and Palm Coast did not incorporate as a city until 1999. Today, by the Census Bureau’s estimates, it holds about 98,000 of Flagler County’s 126,500 residents. Our office is at Nocatee Town Center; for the team, the services and how we work, start with home care across Ponte Vedra.
The Census Bureau’s 2020–2024 American Community Survey estimates describe two different ZIP codes. 32137 is the older one, with a median age of 56.9 against 53.1 for Flagler County, about 16,900 of its roughly 48,400 residents 65 or older, and 53.7% of its households including someone that age. 32164 is younger on paper, with a median age of 47.4 across about 56,700 residents, but it still counts about 15,600 people 65 and older, some 1,800 of them 85 or older. Between them, the two ZIP codes hold about 6,100 residents 65 and older who live alone, nearly four in five of the county’s roughly 7,800. For many of them, the help that matters is ordinary and regular: hourly care for the groceries, laundry, meals and a few hours of company each week, and personal care for bathing and dressing once a shower or a zipper has become the hardest part of the morning.

Two AdventHealth Hospitals, Eight Miles Apart
Flagler County’s two hospitals are both in Palm Coast, and both are AdventHealth. The older one, the 99-bed AdventHealth Palm Coast at 60 Memorial Medical Parkway in 32164, sits off State Road 100 just west of I-95, and the state’s Agency for Health Care Administration lists it as a Primary Stroke Center. AdventHealth Palm Coast Parkway opened on August 2, 2023, at 1 AdventHealth Way in 32137, on Palm Coast Parkway just west of Belle Terre Parkway, with 100 beds and a 24-bed emergency department; it doubled the county’s inpatient capacity. Which one your parent comes home from often follows the side of town. In free-flowing traffic, the Parkway hospital is about 9 minutes’ drive from Matanzas Woods Parkway and about 14 from Washington Oaks in the Hammock, while the State Road 100 campus is about 6 minutes from Seminole Woods Boulevard. When a hospital stay ends in short-term rehab instead, the two nursing homes Medicare lists for Flagler County are Aviata at Grand Oaks on Palm Coast Parkway SE and Flagler Health and Rehabilitation Center in Bunnell.
Discharge can come with a few hours’ notice, from either campus or from rehab, and the first days at home are when a family most needs a second pair of hands. Our hospital-to-home care puts someone in the house the day your parent comes back: prescriptions picked up on the way, meals that follow the discharge instructions and a ride to the first follow-up. Mobility care covers the walks a physical therapist asks for and the trips between bed, bathroom and chair, and overnight care keeps someone awake in the house through the first nights back. If the doctor orders home health nursing or therapy, our caregivers cover the hours between those visits. We are a non-medical, private-pay agency: we accept long-term care insurance, ask for no deposit or long-term contract, and don’t bill Medicare or Medicaid. Care starts with a free in-home assessment and a written plan, and there is a registered nurse on our staff.
Services offered in Palm Coast, FL
Live-In Care
Live-in care provides seniors with the consistency of having the same dedicated caregiver daily, allowing them to build trust and familiarity in their care routine. This arrangement enables seniors who need assistance with most activities of daily living to remain comfortably at home, maintaining their independence while receiving comprehensive support from their live-in caregiver.
Hourly Care
Hourly care provides flexible, scheduled support for seniors in their homes. Caregivers visit for set periods, offering assistance with daily tasks, personal care, and companionship tailored to the client's specific needs and preferences. This service can range from short shifts a few days a week to full 24/7 around-the-clock care, accommodating a wide spectrum of care requirements and ensuring seniors receive the right level of support.
Overnight Care
Overnight care offers peace of mind with an awake caregiver present during nighttime hours. This service ensures assistance is available for nighttime needs, emergencies, or early morning routines, allowing clients needs to be met overnight, enabling family members or daytime caregivers to have uninterrupted sleep.
Always On Virtual Care
Always On Virtual Care pairs our caregivers with Sensi, an AI-powered audio sensor that detects falls, distress, early UTI signs, and other care events around the clock. Sensi alerts our team the moment something changes — so families gain a second set of eyes during the hours a caregiver isn't physically in the room, and seniors stay safer at home for longer.
Storm Season: Canals, Bridges and PEP Tanks
When Hurricane Matthew came up the coast in October 2016, Flagler County ordered everyone east of the Intracoastal to evacuate, the Hammock included, along with all of the C Section and parts of the F Section, and warned that once sustained winds reached 45 mph the bridges would close and first responders would not be able to respond. At 6 p.m. on October 7, as the storm passed, the National Weather Service recorded a storm surge 6 feet deep in the Palm Coast saltwater canal, about 44 inches over the bulkhead. Hurricane Milton, in October 2024, brought rain instead: between October 5 and 11 the city measured 16.06 inches in its southwest and 13.53 in the west, and its utility department sent tank trucks to the Woodlands to pump water off impassable roads. Since 2024 the county has not used lettered evacuation zones, partly because Palm Coast’s neighborhoods already go by letter. It now maps a Potential Hurricane Evacuation Zone on the east side of the county and, when it orders an evacuation, publishes a separate map of the neighborhoods that must leave. Look up your parent’s address on the county’s Hurricane Evacuation Planning Zones map rather than going by the section letter.
Losing power brings a problem particular to Palm Coast. In neighborhoods on the city’s PEP sewer system, each house has a buried tank and a small electric pump, and without power the tank holds about a normal day’s use; after that, wastewater can back up into the house, usually in the shower first. The city asks those households to skip laundry and the dishwasher, cut back on showers and flushing, and call utility customer service, not a plumber, if the red alarm light comes on. On October 10, 2024, after Milton, more than 50,000 Flagler County customers were still without power. Residents who depend on electricity for life-supporting equipment, or who need help with basic everyday tasks, can register with Flagler County Emergency Management’s special needs program at (386) 313-4200, but special needs shelter staff do not give one-on-one care, and the county asks that caregivers come along. With live-in care, someone is already in the house when a watch is posted: packing medications and paperwork, keeping water use down while the pump is out, and making it possible to leave early, before the bridges close. For the coast south of the Hammock, see home care in Flagler Beach.
Rides, Meals and Day Programs in Flagler County
Flagler County Public Transportation runs shared-ride, door-to-door trips for $2 each way, for residents with little other way to get around who are over 60, have a disability that keeps them from driving, or have a low income. Riders must apply and be approved before the first trip, new bookings are taken at (386) 313-4100, the county suggests booking four to five days ahead, there is no same-day service, and drivers wait three minutes at the pickup before moving on. It does not leave the county, so a specialist in Daytona Beach or Jacksonville needs another ride. Companion care fills those gaps: a caregiver who drives to the appointment, waits, takes notes and picks up the prescription, or who simply has your parent ready at the door when the county bus pulls up. The county’s senior center meets at the Nexus Center on Commerce Parkway in Bunnell, about 9 minutes’ drive from Seminole Woods, on weekdays, with a hot lunch, exercise, line dancing and bingo (see home care in Bunnell). For a parent who can no longer cook, the county’s Meals on Wheels delivers frozen meals each week to eligible residents over 60.
For a parent with memory loss, Alzheimer’s and dementia care keeps a weekday rhythm going at home: the same morning routine, lunch at the same hour, a walk or an outing, and a familiar face instead of a new building. The county’s subsidized in-home services, provided through contracted agencies, start with a screening by the Aging and Disability Resource Center at 1-888-242-4464, and the county notes that waitlists, fees and service restrictions may apply; private-pay care can cover the wait.
