Home care in southeast Jacksonville
Dependable caregivers for older adults in Jacksonville ZIP codes 32224, 32225 and 32256, around Mayo Clinic and UNF, along Kernan and Hodges, south through Baymeadows, Deerwood and Bayard, and north to Fort Caroline and Queen's Harbour. Our Nocatee office is next door, and a call or text reaches us any time.
Three ZIP codes, not the whole city
Our home care in Jacksonville covers three ZIP codes. 32224 runs from the San Pablo area south across Butler Boulevard and takes in Mayo Clinic, the University of North Florida and the neighborhoods along Kernan and Hodges boulevards. 32256 is the stretch below it: Baymeadows and Deerwood, the Gate Parkway corridor, and farther south eTown and Bayard.
32225 is to the north, in east Arlington: Fort Caroline, Girvin, Hidden Hills and the waterfront streets of Queen's Harbour. St. Johns Town Center, just north of Butler, is in 32246 and outside our area.
32224 and 32256 border ZIP 32081, where our office is, at Nocatee Town Center, so our team is close by when you need us, and the free assessment happens at your loved one's house. For our other Duval County towns, see Atlantic Beach, Neptune Beach and Jacksonville Beach.
Living a few minutes from Mayo Clinic
For families in 32224, Mayo Clinic on San Pablo Road is the neighborhood hospital as well as a destination. Its Memory Disorder Clinic in Jacksonville evaluates memory and thinking problems and offers caregiver education and support groups.
After a diagnosis, most of the work happens at home. Alzheimer's and dementia care keeps the day calm and predictable, with the same familiar faces, meals at the same hours and fewer surprises. When a stay at Mayo Clinic Hospital ends, hospital-to-home care covers the hours between the follow-ups: help to the bathroom and with bathing, clean sheets, meals that fit the discharge instructions and a call to you when something seems off.
UNF is in 32224 too. Its Osher Lifelong Learning Institute runs classes for people 50 and older at the university center near Butler and Kernan, the kind of weekly outing companion care keeps going after a parent stops driving.
Rehab and hospitals around Baymeadows and Deerwood
Just north of 32256, Ascension St. Vincent's Southside on Belfort Road is a Primary Stroke Center with a 24-hour emergency room, and Baptist Medical Center South is a short drive down Old St. Augustine Road. After a stroke, a brain injury or a serious illness, some patients go on to Brooks Rehabilitation Hospital Bartram Campus off I-95 for intensive therapy before they come home.
Coming home from rehab is when families find out how much help the house needs. Mobility care carries the therapist's walking plan into the rest of the week, so the walker gets used between sessions and not just during them. Personal care helps with bathing and dressing for as long as your loved one needs it, and specialized complex care is there when several conditions have to be managed at once.
Buses, paratransit and a storm plan inland
JTA's fixed-route buses are free at 65 with a Florida driver's license, a Florida ID or a Senior STAR Card. For a parent who can't manage a bus stop, Connexion, JTA's paratransit service, runs shared door-to-door rides in Duval County for riders certified under the ADA or the Transportation Disadvantaged program, booked one to seven days ahead at (904) 265-6999, $3 each way for ADA riders.
Both work best with someone on the other end. A caregiver can have your loved one ready for the pickup, or skip the wait and drive them, stay through the appointment and bring them home.
Hurricane zones reach inland here too. Duval County letters them A through E, with Zone F for areas likely to flood from rain, and every mobile home counts as Zone A. Look up your parent's address on JaxReady, and if they use oxygen or powered equipment, put them on the Special Medical Needs Registry at (904) 255-3172. If your parent can't be alone through a storm and blackout, 24-hour care keeps a caregiver awake until the power is back.
Apartments, gated streets and outings near Deerwood
In these ZIP codes, a parent might be in an apartment or condo near Baymeadows, a house in a gated neighborhood off Deerwood or a home near UNF. In an apartment, the challenge is often the long walk from the parking lot. In a house, it's the laundry, the yard and a tub. A caregiver can help with all of it.
The Southeast Regional Library is a good place to begin a weekly outing, with books and a quiet table by the window. With companion care, a caregiver drives, stays and handles the other stops on the list. Soon after a discharge, those familiar stops are what make a week feel ordinary again.
With home care in Jacksonville, your loved one still sees friends, whether that's lunch at a favorite table, a Sunday service or grandchildren waving from a tablet screen. Our Ponte Vedra office sits just over the county line and sends family a word after visits, which helps on the long workdays.

What's the difference between home care and home health care?
Home care, also called non medical home care, helps your loved one with everyday activities so they can stay safe and comfortable at home. A trained caregiver or home health aide assists with personal care like bathing, dressing, and toileting, along with daily tasks such as meal preparation, light housekeeping, errands, and companionship.
Home health care is skilled medical care provided by a registered nurse (RN) or licensed practical nurse (LPN). It covers clinical tasks that require a nurse's oversight, such as IV therapy, wound care, and medication administration. Home health care is usually ordered by a doctor for a set period of time, often after a hospital stay.
Many families use both, with home care filling in the daily support between skilled nursing visits.
How quickly can care start?
In urgent situations, we can often start care the same day. For most families, care begins within one to two days. That gives us time to complete your free in home assessment, create a custom care plan for your loved one, and carefully match them with the right caregiver. We move quickly without cutting corners, because your loved one's safety and comfort always come first.
What happens during the free in home assessment?
During the free in home assessment, we take the time to get to know your loved one and your family, including their daily routines, preferences, and what matters most to them. We also complete a home safety assessment and share any recommendations to help prevent falls and make the home safer, at no charge. From there, we create a custom care plan and help you decide on the home care options and schedule that best fit your loved one's needs.
Will my loved one have the same caregiver each time?
We always do our best to keep the same caregiver with your loved one, and we note your preferences so every visit feels familiar and comfortable. Thanks to our low caregiver turnover and excellent training, most of our clients see the same friendly faces week after week. Of course, caregivers need time off and get sick sometimes too. When that happens, we let you know right away and send a wonderful, fully trained replacement who is already familiar with your loved one's care plan. That's the benefit of working with an agency. You're never left without help.
How much does home care cost?
The cost of in home care depends on several things, including where your loved one lives, how many hours of care they need, how often visits are scheduled, and the type of care required. Because every family's situation is different, the best way to get an accurate price is through our free in home assessment. There's no obligation, and we'll create a care plan that fits your loved one's needs. You'll always have a clear, written estimate before care begins, with no surprises.
How do I reach someone after hours or on weekends?
We're available 24 hours a day, 7 days a week, including nights, weekends, and holidays. Just call our main office number and ask to speak with someone. A real person from our team will help you right away, whether it's a schedule change, a question, or an unexpected situation. We're always here for you.

What families are saying
We brought Always Responsive Home Care into our home to care for my husband, and their caregiver, Adwoa, quickly became part of our family. She was gentle and patient with him, and she always treated him with dignity. She noticed the little things, like how he liked his coffee and when he needed a rest before he said a word. The office team was just as wonderful. Every time I called, someone answered and took care of what we needed. Knowing he was in such good hands gave me peace of mind I hadn't felt in a long time. Thank you, Adwoa and the whole Always Responsive team. We are so grateful.
